The growing exposure to patients’ social difficulties has prompted initiatives within the public health facility. An innovative partnership between the health facility and community pharmacies has been established to serve vulnerable patients receiving psychiatric care. In practice, medications are dispensed by community pharmacies located near the hospital facilities, with no upfront costs to the patient and within a short timeframe.

• This contribution reinforces the role of pharmacists—both hospital and community—in protecting public health and highlights their central and legitimate position to positively influence the overall care of patients. The interest generated by this work attests to its significance, which, beyond medication management, addresses a health need and contributes to a significant evolution in professional practices.
